Pediatric BMI-for-age percentile and weight-based sedation dosing reference
*Guidance only. This calculator is a reference aid. It does not replace clinical judgment, a doctor's order, or your practice's own sedation protocol. Confirm every dose before administration.
*Where these numbers come from. BMI-for-age percentiles use the CDC BMI-for-age growth reference for ages 2 through 19, including the CDC extended BMI method at or above the 95th percentile. Ideal body weight uses the CDC 50th percentile weight-for-age. Dose ranges and maximums follow the sedation protocol supplied by the practice, not the CDC.
BMI and the CDC BMI-for-age percentile calculate automatically from sex, date of birth, measurement date, weight, and height. The percentile applies to children ages 2 through 19 and uses the CDC sex- and age-specific LMS reference values. For results at or above the 95th percentile, the CDC extended BMI-for-age method is used.
β Combination therapy: When using multiple sedative medications, adjust individual doses as appropriate to account for additive or synergistic effects.
Midazolam IV injectable: for oral use only. This is the injectable formulation given by mouth, not intravenously.
